COVID-19 VACCINATION VOLUNTEERS 

Thank you for your interest in serving as a COVID-19 Vaccination Clinic Volunteer to assist the Florida Department of Health. Please complete the form below so that you can register to be a volunteer. Vaccination Clinics generally occur on Tuesdays, Thursdays, and Saturdays from 9 a.m. – 2 p.m. with volunteers arriving at 7 a.m. for orientation and assignment. Volunteers could possibly assist with traffic management, distribute/collect vaccination forms, equipment sanitation and data entry (will be limited off site availability). Please note volunteers will be contacted if additional assistance is needed by FDOH.
